Seven aspects reviewed were, 1) problems in the medical reimbursement system, 2) lack of proper education and practice of informed consent in medical education, 3) lack of well-organised team medicine, 4) lack of well-structured community and regional medical systems, 5) historical and cultural paternalism of the medical profession, 6) dependent attitude of patients, and 7) lack of utilitarian recognition of informed concept as a rigid procedure for making scientific and accountable decision making which enhances and ensures appropriate medication. Informed consent in Japan tends to be recognised as a moral issue related to human right and to medical lawsuits rather than the reasonable decision-making process in medicine. Relations of informed consent with evidence-based medicine and with medical ethics as a science of decision-making in Western countries have been rarely recognised.
