@article{oai:nagasaki-u.repo.nii.ac.jp:00026816, author = {海部, 佳純 and 石松, 祐二 and 松浦, 江美 and 神津, 玲 and 澤井, 照光 and 花田, 匡利 and 原, 敦子 and 城戸, 貴志 and 石本, 裕士 and 坂本, 憲穂 and 尾長谷, 靖 and 田中, 宏典 and 迎, 寛}, journal = {保健学研究, Health Science Research}, month = {Jul}, note = {【目的】近年,慢性閉塞性肺疾患(COPD)における身体活動(PA)の重要性に関する報告は多いが間質性肺疾患(ILD)は十分でない.本研究の目的はILDにおけるPAの特性及びPAに関連する要因を明らかにすることである. 【方法】修正MRC息切れスケールグレード 1(mMRC 1)以上の呼吸困難を呈する安定期ILD群16例,COPD(対照疾患)群14例を対象に,3 軸加速度計(Active style Pro)を用いてPA量(歩数/日,座位/時間,低強度/時間,中強度以上/時間)を測定し 2 群間で比較した.さらに両群のmMRC別(mMRC 1 と 2 )による層別解析を行った.また,ILD群においてPAと関連する要因を検討した. 【結果】両群間で,mMRC,The Nagasaki Respiratory ADL questionnaire(NRADL)得点,6 分間歩行距離(6MWD),PAに有意な差はなかった.層別解析ではmMRC 1 では両群のPaO2 や6MWDは同程度だが,COPD群よりILD群でPAが高い傾向だった.mMRC 2 ではCOPD群よりILD群でNRADL得点,PaO2 や6MWDは有意に高かったが,IP群でPAの著明な低下を認めた.また,ILD群において歩数/日とはBMI,NRADL,%肺拡散能,左握力で正の相関を,mMRCとは負の相関を認めた. 【結論】ILD患者のPAはmMRC 1 でCOPDより高いが,mMRC 2 では著明に低下し,BMI,NRADL,%肺拡散能,握力,ならびに呼吸困難との関連が示唆された. 【Objective】 There are many reports on physical activity( PA) in cases of chronic obstructive pulmonary disease (COPD), but few in cases of interstitial lung disease (ILD). The purpose of this study was to reveal the characteristics and relevant factors of PA levels in patients with ILD. 【Method】 We enrolled 16 patients for the ILD group and 14 patients for the COPD group with dyspnea of modified Medical Research Council( mMRC) grade 1 or higher and compared their PA levels measured using a 3-axis accelerometer( Active style Pro). In addition, we conducted a stratified analysis for mMRC grades 1 and 2 and investigated the correlational factors of PA levels in the ILD group. 【Results】 There was no significant difference in mMRC grades, Nagasaki University Respiratory Activities of Daily Living( NRADL) scores, 6-minute walking distances( 6MWDs), and PA levels between both groups. The stratified analysis revealed that, among those with mMRC grade 1 dyspnea, the PA level in the ILD group was higher than that in the COPD group, although the PaO2 and 6MWDs were similar. Among those with mMRC grade 2 dyspnea, the PA level in the ILD group was as low as that in the COPD group, although the NRADL score, PaO2, and 6MWD in the ILD group were higher than those in the COPD group. In addition, this study showed that the number of daily steps was positively correlated with the body mass index, NRADL score, % diffusing capacity of the lungs for carbon monoxide, and lefthand grip strength and negatively correlated with the mMRC grade. 【Conclusion】 Among patients with mMRC grade 1 dyspnea, the PA level was higher in the ILD group than in the COPD group. However, among patients with mMRC grade 2 dyspnea, the PA level in the ILD group was as low as that in the COPD group.
